食管癌中环氧化酶-2表达与临床病理特征相关关系的Meta分析

Relationship between COX-2 expression and clinicopathologic features of esophageal cancer: a Meta-analysis of case-control studies

摘要: 目的: 系统评价国内有关环氧化酶-2(cyclooxygenase-2,COX-2)表达与食管癌发生、发展的相关关系。方法: 计算机检索CBMdisc(1978~2011)、CNKI(1979~2010)、重庆维普数据库(1989~2010)、中华医学会数字化期刊(1998~2010),并辅以文献追溯及人工检索的方法,收集国内公开发表的所有相关病例对照研究。按纳入、排除标准筛选文献,并按质量评价标准评价纳入研究的质量。应用Revman 5.0 软件进行Meta分析。结果:共纳入7篇病例对照研究,其中食管癌424例,正常食管对照108例。Meta分析结果显示,COX-2在食管癌组和正常食管组表达的差别有统计学意义[OR=31.44,95%CI(10.47,94.38),P<0.01]。COX-2在食管癌高、中分化组和低分化组表达的差别有统计学意义[OR=3.60,95%CI(1.63,7.99)]。而COX-2在性别、年龄、浸润程度、淋巴结转移方面的OR值分别为1.56(0.61,3.98)、 0.59(0.27,1.30)、1.21(0.49,2.98)、1.47(0.38,5.64),其差别无统计学意义(P>0.05)。 结论: 根据现有的国内研究证据提示,COX-2高表达与食管癌及其病理分化程度有关。

Abstract: OBJECTIVE: To explore the association between COX-2 expression in human esophageal cancer and its clinicopathologic characteristics of esophageal cancer. METHODS:Published studies were searched in the CBMdisc(1978-2011),CNKI(1979-2010),VIP(1989-2010) database,CMA Digital Periodicals(1998-2010),other relevant journals were manually searched to identify all the related case-control studies in mainland China. The quality of the included studies was assessed. Cochrane Collaboration’s Software Revman 5.0.2 was utilized to test the heterogeneity, the overall effect and the publication bias of the combined studies as well. RESULTS:Seven studies were included. For the positive rate of COX-2 expression,significant difference was tested between esophageal cancer and normal esophageal tissues[OR=31.44,95%CI(10.47,94.38)]. In addition,cell differentiation G1+G2 vs. cell differentiation G3 revealed significant difference[OR=3.60,95%CI(1.63,7.99)]. However,there was not any significant difference shown in terms of gender,age,depth of invasion,lymph node metastasis,of which the overall effect were 1.56 (0.61,3.98),0.59(0.27,1.30)、1.21(0.49,2.98),1.47 (0.38,5.64),respectively. CONCLUSION:According to the currently available national evidence,higher COX-2 expression may be associated with esophageal cancer and cell differentiation.
